Gall Bladder Surgery

The function of the digestive system is aided by the gallbladder, an organ under the liver. It is attached to the bile duct by the cystic duct which moves on to the small intestine and liver. The gall bladder stores a small quantity of bile. Gall bladder diseases generally occur when the bile gets solid due to chemical imbalance. Sometimes stones are found in the bladder which traps the bile in the bladder. Symptoms of pain in the upper abdomen and vomiting appear which may be mistaken as symptoms of other ailment.

Apollo Hospital Medical and Surgical Gastroenterology treats patients with gallbladder disorders. It has the country's beat doctors and surgeons treating gallbladder diseases.

Diseases of the Gall Bladder
Cholecystitis
Stone diseases
Cancer of the bladder

Cholecystitis
When the abdominal lining or the gallbladder wall swells Cholecystitis is suspected. It is caused by the presence of a gallstone in the cystic duct. Besides, there are other causes of Cholecystitis too. Bile duct infected by bacteria, pancreas or liver tumor, less blood supply to the gall bladder and sludge present in the gallbladder.
Diagnosis of the disease can be done by sonography, hepatobiliary scintigraphy, cholangiography, percutaneous transhepatic cholangiography, endoscopic retrograde cholangiopancreatography and computed tomography scan.

Treatment
Treatment of Cholecystitis sometimes involves surgical removal of Gall Bladder.

Gallstones

Bile that is stored in the gall bladder to aid digestion contains water, bile salts, proteins, cholestrol and bilirubin. When the liquid bile contains too much of bilirubin, cholestrol and bile salts sometimes it hardens into stones. Pigment stones and Cholesterol stones are the two types of gallstones found in the duct. Gallstones can prevent the free flow of bile to the ducts. Thus bile locked in the ducts can give rise to inflammation of the ducts, gallbladder or sometimes the liver. If a gallstone blocks the opening of the pancreatic duct, digestive juices trapped in the pancreas can lead to painful swelling called gallstone pancreatitis.

Treatment
Surgery of the gallbladder is the most common way to treat gallstones. The surgery is called cholecystectomy. Tiny incisions are made in the abdomen and surgical instrument with miniature video camera is inserted. The surgeon gats the magnified image in the monitor which aids him to perform the operation. This is a laparoscopic surgery which minimizes post operation pain.
When the surgeon meets with obstacles in the laparoscopic surgery, such as infections, open surgery is undertaken. The common complication in a gallbladder surgery is the injury caused to the bile ducts. When bile leaks dangerous infection occurs which again needs additional surgery.

When gallstones are found in the bile ducts, a gastroenterologist uses endoscopic retrograde cholangiopancreatography (ERCP) to find and remove them during or before the surgery.

Cancer of the bladder

The causes of gallbladder cancer is not precisely known but people with a history of gallstones or those who have polyps or tumors in the bladder are likely to have cancer. People born with congenital abnormalities or having a porcelain gall bladder with calcium forms in the wall can be prone to cancer of the gall bladder as well.

Diagnosis of Gallbladder cancer can be done through the following ways:
Ultrasound scan
CT (computerised tomography) scan
Spiral CT scan
MRI (magnetic resonance imaging) scan
ERCP (endoscopic retrograde cholangio-pancreatography)
Angiogram
Laparoscopy
Laparotomy

Surgery
When cancer has not spread beyond the area of gall bladder surgery is the best option . In case of cancer affecting other areas surgery can be done and well as other ways to improve the patient's condition is undertaken.

Radio therapy, Chemotherapy and Stent insertion are the other methods to combat cancer being spread in the body.
