Different types of cataract surgery
There are two types of cataract surgery. Your doctor can explain the differences and help determine which is better for you:

  • Phacoemulsification or phaco: A small incision is made on the side of the cornea, the clear, dome-shaped surface that covers the front of the eye. Your doctor inserts a tiny probe into the eye. This device emits ultrasound waves that soften and break up the lens so that it can be removed by suction. Most cataract surgery today is done by phacoemulsification, also called "small incision cataract surgery."

Phacoemulsification is the most modern method of removing a cataract lens. This surgery is done under topical anaesthesia and no stitches are applied. This greatly reduces trauma to the eye.

  • Extracapsular surgery. Your doctor makes a longer incision on the side of the cornea and removes the cloudy core of the lens in one piece. The rest of the lens is removed by suction. After the natural lens has been removed, it often is replaced by an artificial lens, called an intraocular lens (IOL). An IOL is a clear, plastic lens that requires no care and becomes a permanent part of your eye. Light is focused clearly by the IOL onto the retina, improving your vision. You will not feel or see the new lens.

    Small Incision Refractive Cataract Surgery:
It is an advanced technique developed to enhance the ability of the surgeon to correct almost all known focusing errors. This can correct the full range of Hyperopia (long-sightedness), Myopia (shortsightedness), Astigmatism, Presbyopia and Spherical Aberration resulting in the best possible quality of vision without glasses or contact lenses.

Sophisticated technology is used to assess the focusing system of the eye prior to surgery. This includes computerised slit scan corneal topography (corneal power/shape mapping), pupillometry (pupil size), and dual linear partial coherence interferometry (the most accurate and reproducible way of measuring the length of your eye). This information is used in combination with an assessment of your visual needs and lifestyle to generate a tailor made Surgical Prescription (surgical plan). Additional surgical techniques to correct astigmatism are combined with state of the art replacement lens technology (Multifocal, Accommodative or Tecnis) to achieve the best possible functional quality of vision without glasses or contact lenses. The surgery is carried out in most cases using eye drop (topical, no needle) anaesthesia. Patients are up and about immediately after surgery and visual recovery is rapid.

Vision Correction Surgery includes a wide range of Laser and Non Laser procedures. Just as a size 14 dress would not fit a size 10 person then a surgical technique that is right for one patient will not be right for another. You may have seen many advertisements depicting Laser eye surgery in a very selective and simplistic style. The fact is that laser vision correction is safe and effective provided the surgery be tailored to every patient rather than offering each patient a single procedure like LASIK. It is like a car salesperson saying, "You can have a car in any colour provided it is black". It is not right when you buy a car and certainly not when it comes to surgery on your eyes.

    Laser Vision Surgery:
In general if if any patient is younger than 50, they do not need reading glasses and have a mild to moderate focusing error. Laser Vision surgery may be the procedure of choice. If you are older than 50, need or are likely to need reading glasses in the near future or have a severe focusing error, Laser vision surgery is generally inappropriate and lens based surgery like Refractive Lens Exchange or Phakic Intra-Ocular Lenses could provide a better solution.

    Presbyopic Lens Exchange
Presbyopic Lens Exchange is a surgical procedure, designed to correct Presbyopia by exchanging the ageing natural lens of the eye for a new intra-ocular lens. This new lens could be either a Multifocal Lens (similar to a varifocal spectacle lens) or an Accommodating lens which mimics the natural lens and moves within the eye to adjust its focusing power depending on whether the eye is looking at distant or near objects. This surgery evolved from Refractive Cataract Surgery where the cataract (cloudy natural lens) is exchanged for a Multifocal or Accommodative lens. Thus, the PRELEX operation is very similar to a cataract operation but uses new replacement lens technology to restore good distance and near vision without traditional bifocal, varifocal spectacles or contact lenses. The vast majority of patients are able to function most of the time without spectacles or contact lenses while needing glasses or contact lenses for some visually demanding activities like driving, reading fine print etc. As the ageing lens is replaced, it eliminates the risk of developing a cataract and in some ways is like bringing forward your cataract operation by a few years.
